v163 - April 30, 2026 Update

We are excited to announce the launch of 4D Scribe within the 4D EMR platform. It is a combined AI scribe and dictation option designed to simplify clinical documentation. In the Scribe mode it records the spoken conversation during the patient encounter into structured chart notes. It significantly reduces time spent on manual charting while maintaining clear and consistent records. For encounters such as operative reports, or for those who prefer dictation,  Dictation mode can process that as well with straight speech-to-text. All offices will have access to a free two-week trial from May 1st through May 15th, during which 4D Scribe will be enabled automatically for all offices with access to patient charting (normally $175/month/user). A separate email will be sent on Friday describing how to use this trial feature. 
In our last release, we announced the upcoming release of Memberships. Memberships provide a streamlined way to enroll patients in recurring packages of services and products. Memberships automatically handle billing the patient's saved credit card, generating the package, and expiring unused items in previous membership installments. Release 163 introduces the next Treatment Table: Neuromodulators. This update makes it easier for providers to document injection details and quickly convert them into chart notes that are automatically signed or routed for co-signature when needed. 

This release also enhances the new patient entry process by automatically checking leads in addition to existing patient records after adding a new patient name, helping to prevent duplicates and improve data accuracy.

Also, the photo canvas now includes optional grid and facial oval overlays to support more consistent alignment and framing for before-and-after photos.

Release Highlights

Treatment Tables - Neuromodulators

Release 163 includes the next Treatment Table option: Neuromodulators. The Neuromodulator Treatment Table can be found by going to Patients > Clinical > Treatment Tables > Neuromodulators. The Neuromodulator Treatment Table can be used to log lot numbers, expiration dates, and the number of units of any injectable configured as a neuromodulator in Practice > Products > Product List. 

The table can be converted into a chart note using the +Chart Note button. These notes are created as stand-alone notes separate from visit notes, and they are automatically signed by the user when created. If the treatment note type is flagged as requiring provider co-signature, they will be sent to the provider for final review.

In order to use the Treatment Table along with Drawings in a chart note, it is recommended to first create the chart note from the Treatment Table. The signed note can be opened and edited to include the stamped drawing.

Convert Lead to New Patient from Add Patient Window

The system now automatically searches both existing leads and patient records when adding a new patient. This helps identify potential matches and prevents duplicate entries, allowing staff to confirm whether a record already exists before creating a new one, improving accuracy and efficiency.

Photo Grid and Facial Oval

The photo canvas now includes optional grid and facial oval overlays designed to improve photo consistency. These guides help position the subject more precisely, ensuring that before-and-after images are aligned, scaled, and framed in a uniform way. This makes it easier to compare results over time while maintaining a more professional and standardized presentation.
