v161 - March 26, 2026 Update

Release 161 introduces our new Treatment Tables feature. Treatment tables are visit logs that track a patient's progress over time. To introduce the feature, we are releasing a Weight Loss Treatment Table. Additional treatment logs will be added in upcoming releases. Treatment tables can be easily converted into signed chart notes to be included with the patient's medical chart.

There’s also a new Schedule Utilization Report that shows how much of a provider’s available time is actually booked, helping you understand and optimize scheduling.

A complete list of new features, improvements, and bug fixes is provided below. If you have any questions, please contact the 4D EMR Support Team at support@4d-emr.com or by calling 844-878-2434.

Release Highlights

Treatment  Tables - Weight Loss

Treatment tables are a new clinical feature designed to help track patient progress over time. The Weight Loss Treatment Table can be accessed by going to Patients > Clinical > Treatment Tables > Weight Loss. Additional treatment tables are planned for future releases.

Each Weight Loss visit can be added to the table using the +New button. The table can be converted into a chart note using the +Chart Note button. These notes are created as stand-alone notes separate from visit notes and they are automatically signed by the user when created. If the Treatment Note type is flagged as requiring provider co-signature, they will be sent to the provider for final review.

Schedule Utilization Report

The Utilization Report can be found in the Scheduler reports section. This report calculates the total number of working hours per provider, either based on appointment placeholders or the office hours, and the amount of time the provider has scheduled appointments to determine the utilization percentage.
