A Quick Guide to Tepe

Uploading Files from EMR software

1.       Open the patient’s profile

2.       Ensure the Patient has an email address on file

3.       Go to Forms & Scans > Generate Forms

4.       Select the Form you wish to send to the patient

5.       When prompted how to generate the form select Text

6.       Select whether to send a link to the patient portal to the patient

a.       Note:  A separate link to the patient portal can be sent later if needed

Accessing TEPE in the Office

1.       On the device the patient will use to sign the form go to https://tepe.4d-emr.com/

2.       Log-in using the same email and password used to access 4D EMR

3.       Select the patient in the list

a.       If the patient has an appointment today, select the provider from the drop down then select the patient from the list of appointments

b.       If the patient does not have an appointment today click on All Patients then search for the patient by name or patient ID

4.       Verify the identity of the patient by asking them to answer one of the displayed questions then click “Next”

5.       Review list of forms waiting for the patient to review and sign.

a.       Click the X on any document to remove it from the portal

b.       Additional documents can be added by clicking the various categories and selecting individual documents

6.       Click Go to access the Patient Portal

7.       Hand the device to the patient

 

Patient Portal

When the patient accesses the Patient Portal either from a link send to them via email or text or through TEPE they will follow this general flow:

1.       The patient will first be asked to confirm their identity. 

a.       If accessing from staff-accessed TEPE they will simply click “Yes that’s Me”

b.       If Accessing from an emailed link they will be prompted to enter in their Date of Birth to confirm identity

2.       Once their identity has been verified the patient will be presented with a list of documents to be reviewed.

3.       Click the Text

4.       The document will then open in a form format which will allow the patient to answer questions, check checkboxes, and add initials or signatures.

5.       After all required signatures and initials have been added the patient can click Text

a.       If the patient has concerns or questions and does not want to sign the form they can click A picture containing text

Viewing Document Status

Signed and reviewed documents can be accessed Forms & Scans > View & Import.  

A listing of any unsigned consents will display under Forms & Scans > Generate Forms. It is recommended to review this list of documents prior to appointments to ensure all required documents have been completed.
