- The SIS (Surgical Information Systems) integration enables automatic and manual data exchange between 4D EMR and SIS, including appointments, patient demographics, and chart notes.
What the Integration Sends to SIS
1. Appointments
- Surgical Appointments are automatically transmitted to SIS when saved.
- Appointments are filtered by the Facility Locations selected in the integration settings.
2. Patient Data
- Patient demographics are sent together with the appointment.
- Patient data is not sent independently—it only transmits when an eligible appointment is sent to SIS.
3. Chart Notes
- Chart notes are sent manually using the Send to SIS button in the Read Chart Notes list in the patient record.
- This button appears only for chart notes linked to eligible appointments when the SIS integration is enabled.
- Chart notes may be sent for:
- Procedure appointments, and
- Pre‑op appointments
If a chart note is sent for a Pre‑op appointment, SIS will automatically associate it with the related Surgical Appointment.
4D EMR Workflow (Internal Instructions)
Integration Configuration
- Navigate to Practice → Settings → Integrations in 4D EMR.
- Select the Surgical Information Systems tile.

- Enter the credentials provided by SIS:
- Host
- Port
- Username
- Password
- Select the Facility Locations that should trigger data transmission to SIS.

- Only appointments for the selected locations will be sent.

- Navigate to Practice → Admin → Appointment Types.
- Edit each appointment type that should sync.
- Check the box for Surgical Appointment.

Automatic Sending of Appointments
When Surgical are created or saved in 4D EMR:
- They are automatically transmitted to SIS.
- Patient demographic data is included with each appointment.
Note: Only appointment types configured as Surgery Appointment and scheduled at a Facility Location that is selected in the Facilities Location list in the SIS setup will be transmitted from 4D EMR to SIS.
Sending Chart Notes
- View the chart note list for a patient by navigating to: Patient → Clinical → Read Chart Note
- Notes linked to eligible appointments will display a Send to SIS button (visible only when the integration is enabled)

- Clicking the Send to SIS button will transmit the chart note directly to SIS.
- If the note is sent from a Pre‑op appointment, SIS will automatically attach it to the corresponding Surgical Appointment.

Receiving Documents from SIS
- 4D EMR automatically pulls documents from SIS.
- Retrieved documents are saved in the Forms & Scans section of the patient record in 4D EMR.
