Description

4D EMR directly integrates data about upcoming surgery cases and receives the full surgical chart after procedure is completed.

Account Integration Setup

  1. Setup account with Sharable Forms www.shareableforms.com.
  2. In 4D EMR, navigate to Practice>>Settings>>Integrations and select the Sharable Forms tile.
  3. Enter the Account ID provided to the practice by Sharable Forms in item (1.) shown in below image.
  4. In item (2.) shown in below image, select the previously configured 4D EMR Facility Locations that require surgical charting through Shareable Forms.
  5. Click the Enable button

4D EMR - Shareable Forms Integration

  1. 4D EMR uploads appointments to Shareable forms when created or updated that meet the following specifications:
    1. Appointment Type set as Surgical Appointment as shown in item (6.) in below image.
    2. Facility location on appointment as shown in item (3.) in below image matches a Facility listed in Facility Locations as shown in item (2.) in below image.

      9. Procedure (from quote linked to the appointment)
      10. Anesthesia (from quote linked to the appointment)
      11. Duration 
      12. Cancelled (status)
      13. Surgeon

    2. Patient Demographic Data 
      1. PatientId (Not visible from user interface - internal code use only)
      2. PatentMRN (4D EMR Patient ID)

  2. Any other information NOT specifically listed above is NOT sent from 4D EMR to Sharable Forms (e.g. Review of Systems and Social History are NOT sent to Shareable Forms).
  3. Surgical Chart Note doucments will then be generated by practice staff through the Shareable Forms application.
  4. 4D EMR downloads completed PDF surgical charts from Shareable Forms every 20 minutes and adds the document to the patient Forms & Scans>>View & Import section.
