Appt Review 

Overview

In earlier modules, we reviewed the basics of how appointments work.

There are many settings available, but you only need to use the features that best support your practice.

Before moving into advanced settings and configuration, this section provides a quick refresher on how to create appointments.

Creating an Appointment from the Schedule

From the schedule:

  1. Right‑click on the desired time slot
  2. Choose New Patient or Existing Patient

If you select Existing Patient, simply search for the patient and select their profile.

Next, choose the appointment type the patient is coming in for.
(We will cover how to edit existing types and create new ones later in this module.)

Using Appointment Subtypes

You can also set up appointment subtypes.
These are optional, but many practices find them helpful.

Example:

  • Appointment Type: Injectables
  • Subtype: Neuromodulator or Filler

Finalizing Appointment Details

Add any necessary comments, then review the appointment details to confirm:

  • Location
  • Date
  • Time

Once everything looks correct, select Save and the appointment will appear on the calendar.

Alternative Way to Create Appointments

Appointments can also be created by selecting the blue calendar icon in the bottom right corner of the page and following the same steps.


 

 

 

 


Appointment Types 

Overview

Here is how to create and edit appointment types.

It’s important to build your appointment types before setting up any other part of your scheduler.

Navigate to:
Practice → Admin → Scheduling → Appointment Types

4D includes default appointment types that you can modify, and you can also create brand‑new ones.


Creating a New Appointment Type

To create a new appointment type:

  1. Select the + New icon near the top of the page

  2. A new page will load where you can build out the details of your appointment type

Let’s walk through each field.


Basic Appointment Type Fields

Name

Enter the name of your appointment type.

Category

This field is very important. Choose whether the appointment is for:

  • A New Patient

  • An Existing Patient

  • A Happening

You cannot schedule an existing‑patient appointment type for a new patient, and vice versa.

If an appointment applies to both new and existing patients, you must create two separate types—one for each category.

Example:
A practice offering laser services would create:

  • Laser – New Patient

  • Laser – Existing Patient

Default Duration

Select how long the appointment should appear on the schedule.
For example, choosing 30 minutes automatically reserves a 30‑minute time slot.

Description

Add internal notes or details about the appointment type.

Schedule Color

Choose the color that will appear on the schedule.
You can use the color wheel or enter RGB or HEX codes.


Chart Note Type

For new appointment types, we recommend selecting History and Physical.

This determines the chart note heading and affects additional features covered later in the Charting module.


Behavior Settings (Toggles)

Collect Co‑Payment

Turn this on only for insurance appointment types, and only if your practice accepts insurance.

This feature is covered in detail in the Insurance module.

Surgical Appointment Type

Use this only for surgery‑related appointments—not for med spa services or consults.

Can Be Outside of the Office

Enable this if the appointment takes place at another facility, such as a hospital or surgery center.

Associated With Quote

Enable this for surgery and pre‑op appointments so the quote can be linked to the appointment.

Use Quote Time

Turn this on for surgery appointments so the scheduler blocks off the correct amount of time.

Show Consult Reasons

Enables a dropdown during scheduling based on the consult reasons configured in the previous module.

Require Card on File

If your practice integrates with a qualified payment processor, you can require a card on file.
Work with onboarding staff to confirm which processors qualify.

Buffer Time

Enable this if you need prep or cleanup time before or after an appointment.
Specify where the buffer applies and how much time to add.


Subtypes

Subtypes allow you to create variations of a single appointment type with different durations.

Example:

  • Halo laser duration differs from BBL duration

  • Create two subtypes under the same appointment type

To create a subtype:

  1. Select + New

  2. Enter the subtype name

  3. Set the duration

  4. Select the blue check mark to save

You can create as many subtypes as needed.
Selecting a subtype during scheduling automatically updates the appointment duration.

Subtypes are optional and should only be used if they support your workflow.


Provider Restrictions

If certain providers should only use certain appointment types, you can set restrictions.

  1. Select + New to the right of Provider

  2. Additional dropdown fields will appear

  3. Choose the provider, duration, and primary provider

This is especially useful for surgery appointment types, ensuring that only medical providers can be scheduled for surgical appointments.

Set these as needed for each appointment type.


Add‑On Calendars

If your practice uses device or room calendars, you can link them here.

  1. Select + New to the right of Add‑On Calendar

  2. Choose the appropriate calendar

Add‑on calendars are covered in detail in other content. 

 

 

Add On Calendars

Overview

Some practices prefer to use additional calendars—such as device or room calendars—to help prevent double booking.

This section walks through how to set up these calendars and how they function within 4D.


Creating a Room or Device Calendar

Navigate to:
Practice → Admin → Scheduling → Calendars

At the top of the page, you’ll see any staff members who already have calendars assigned.

To create an additional calendar:

  1. Go to the Other Calendar section

  2. Select + New

  3. A new window will appear

Example:
You may need a calendar for a laser room to ensure the room is not double booked.

Enter the following:

  1. Name for the calendar

  2. Background color

  3. Whether the calendar should be visible to all staff

Once all fields are complete, select Save.


Viewing and Using the Calendar

When you return to the schedule, you’ll see your new room or device calendar available to toggle on or off.

This allows your practice to:

  • Schedule appointments

  • Assign them to a specific room or device calendar

  • Track both provider and resource availability


Assigning Appointments to a Calendar

When creating an appointment, you can manually assign it to a calendar:

  1. Use the Schedule dropdown

  2. Select the appropriate room or device calendar


Automating Calendar Assignment with Appointment Types

You can automate this process so certain appointment types always use a specific calendar.

Navigate to:
Practice → Admin → Scheduling → Appointment Types

Then:

  1. Select the appointment type you want to configure

  2. Scroll to the Add On Calendar section

  3. Select + New

  4. Choose the calendar you want this appointment type to automatically use

  5. Save your changes

Now, when scheduling this appointment type—with the Add On Calendar toggle enabled—the appointment will automatically be assigned to the linked calendar.


 

 

 

Placeholders 

Overview

4D allows practices to use appointment placeholders to add more structure and control to their schedules.

Placeholders indicate what type of appointment should be booked in each time slot and prevent users from scheduling over those times.

This feature is completely optional. If placeholders don’t fit your workflow, you can skip this section.


How Placeholders Work

Before using this feature, ensure all of your appointment types are already created.

On the schedule, you’ll see blocked time slots showing what appointment should be booked during those periods.

When you right‑click on a placeholder and select Create Appointment, the system will auto‑populate as much appointment information as possible.

  • Search for and select the patient

  • Save the appointment

The patient is now scheduled directly into the designated placeholder slot.


Creating Placeholders

Start on the Schedule page in 4D.

  1. Select the gear icon in the top right

  2. Choose Manage Placeholders from the dropdown

The schedule will temporarily hide all appointments so you can focus on building placeholders.


Adding a Placeholder

Right‑click on the time slot where you want the placeholder and select Create.
A new window will appear.

Enter the following:

  1. Title for your placeholder (any naming convention that fits your practice)

  2. Provider who will use this placeholder

  3. Appointment type

    • Expand categories by clicking the arrows next to New Patient or Existing Patient

    • Select the appropriate appointment type and complete any required details


Repeating Placeholders

If the placeholder needs to repeat:

  • Adjust the Repeat dropdown to Weekly, Monthly, or Yearly

  • Example:

    • To repeat every Monday, Wednesday, and Friday

    • Leave Repeat every set to 1

    • Select Monday, Wednesday, and Friday in the Repeat on section

You can also:

  • Set an end date

  • Exclude specific dates as needed

Select Save when finished.


Stacking Placeholders

You can stack placeholders by right‑clicking in the white space to the right of the schedule.

This allows you to:

  • Overlap placeholders for the same time block

  • Prepare for multiple simultaneous appointments


Returning to the Regular Schedule

When you’re finished:

  1. Return to the gear icon

  2. Select Show Appointments

This switches you back to the regular schedule view.


 

Templates

Overview

Schedule templates allow you to control what types of appointments can be booked during specific time frames, prevent double booking of shared resources (such as portable devices), and communicate provider availability.

Before we walk through how to configure templates, here are brief instructions of how they look and function.

If your practice doesn’t need scheduling templates, feel free to skip this section.


How Templates Work

After templates are set up, you can color‑code different times on the schedule to indicate what appointment types should be booked during those periods.

If you attempt to schedule an appointment that is not allowed by the template:

  • A warning appears

  • The system notifies you that this appointment type is not usually scheduled at that time

  • You may override the warning when exceptions occur, but this should only be done with provider approval

Templates can also help prevent double booking, especially when paired with add‑on calendars.

For example, if a device is already in use, scheduling another appointment requiring that device will trigger a warning.

This is the basic functionality of schedule templates.


Accessing Template Settings

To begin creating templates:

  1. Select the options gear icon near the top right

  2. Choose Show Templates from the dropdown

This hides all existing appointments so you can focus on building templates.


Creating a Template

  1. Right‑click on the time and calendar where you want to create a template and select Create.
  2. Before placing a template on the calendar, you must create a profile.
  3. Select + New to the right of the Profile field

  4. Give the template a name

    • Example: Laser Device Template

  5. Choose a background color for the template. 

We recommend toggling Show Template Name to help prevent double booking and make template usage clear.


Selecting Allowed Appointment Types

Next, choose which appointment types should be allowed within this profile.

  • Expand the New Patient or Existing Patient categories

  • Select the appointment types that should be scheduled during this template

Example:
If this is a laser template, select only your laser‑related appointment types for both new and existing patients.

Alternatively, you can block all appointments if you don’t want any bookings allowed during this time—useful for out‑of‑office templates.


Preventing Double Booking

If you need to prevent simultaneous bookings:

  • Set the Max Simultaneous Bookings value

  • You may also designate appointment types that should ignore these limits

Example:
For a laser template, set the maximum to 1 to prevent double booking of the laser room.

Choose whether this profile applies to all locations or only one.

Once saved, your template profile is ready for use.


Applying the Template to the Schedule

Now configure when the template should appear on the schedule.

Set:

  • Start and end time

  • Start and end date

  • Days of the week the profile applies

You can also:

  • Choose which weeks of the month the profile repeats

  • Set the interval for repetition

  • Exclude specific dates if needed

Select Save when finished.


Viewing Templates in Action

When you return to the schedule, you’ll see the template applied.

If you try to book an appointment type that isn’t allowed:

  • The system warns you

  • You can choose whether to override the restriction

This provides flexibility when exceptions are necessary.


Using Multiple Templates

You can mix and match different template profiles each day to meet your practice needs.

Templates can also be assigned across:

  • Multiple providers

  • Multiple calendars

This gives you greater flexibility in organizing schedules and managing resource usage.


 

Fine Tuning Appointments 

Overview

So far, we’ve covered appointment types, placeholders, calendars, and templates in depth.
This section highlights additional appointment‑related features available in 4D that help you fine‑tune your scheduling workflow.

Using Happenings to Block Time

If you need to block off time on your schedule and do not want to use templates, you can use Happenings.

Happenings block off the schedule for non–patient‑related time.

To create a Happening:

  1. Right‑click on the schedule
  2. Select Happening
  3. Enter the subject, add comments, and adjust start and end times

You can create multiple Happening types if needed.

Repeating Happenings

You can repeat Happenings:

  • Weekly
  • Monthly
  • Yearly

Select the repeat interval, then configure the pattern.

Example:
To block off lunch every Monday, Wednesday, and Friday:

  • Select Weekly
  • Repeat every 1 week
  • Choose Mon, Wed, Fri
  • Set an end date if needed

Select Save to apply the repeating Happening to the calendar.

Viewing the Full Schedule

If you need a complete view of the day:

  • Select Show 24 Hours under the options gear

You can also activate Show Inactive to see where cancelled or rescheduled appointments originally existed.

Using Room Manager

Select Room Manager near the top of the page to view a visual representation of:

  • Appointments for the day
  • Whether a patient has been roomed, seen, or checked out

If Online Bookings are enabled, you can also move these appointment bubbles as needed.

Online Booking Requests

An Online Bookings icon appears near the top middle of the page.
This is where you’ll be notified of incoming online booking requests.

(Online booking setup and workflow are covered in a separate video.)

Zooming the Schedule

Use the magnifying glass slider to zoom in or out of your schedule.
This allows you to expand or compress the view to fit your practice needs.

Consult Reasons

Consult Reasons were covered in the Portal module, but here’s a brief reminder.

When creating a consult appointment, you can choose from a list of consult options—if configured.

To add or remove Consult Reasons:

  • Navigate to Practice → Admin → Doctors and Staff
  • Select the calendar icon to the left of the provider’s name
  • This is where your Consult Reasons live
  • Add or modify them as needed

Summary

Use these additional tools such as Happenings, Room Manager, Online Bookings, zoom controls, and Consult Reasons to fine‑tune your scheduling workflow.

Configure these features in the way that best supports your practice’s needs.

 

Chart Note Type Basics

Overview

Under each appointment type, there is a dropdown for selecting the Chart Note Type that will be used when documenting that appointment.

When first building your appointment types, we recommend setting all of them to History and Physical until you have your charting structure finalized.

This section covers the basics of where to add new Chart Note Types and how to link them to your appointment types.

All other chart‑note‑related settings will be covered in the Charting module.

For now, remember:

The Chart Note Type determines the heading that appears at the top of your chart note.

Adding New Chart Note Types

Navigate to:
Practice → Settings → Chart Note Types

For this module, we recommend following three steps:

Step 1 — Delete Unneeded Chart Note Types

  • Hover to the right of any note type you do not need
  • Select the trash can icon to delete it

This helps keep your list clean and relevant.

Step 2 — Add Missing Chart Note Types

  • Select + New to add any Chart Note Types your practice needs
  • Do not toggle any of the additional boxes yet
  • These settings will be explained in the Charting module, once you have more context about how they function

At this stage, you are simply building your list of available note headings.

Step 3 — Link Chart Note Types to Appointment Types

After creating your new note types:

  1. Navigate to Practice → Admin → Scheduling → Appointment Types
  2. Select the appointment type you want to update
  3. Locate the Chart Note Type to write for this appointment dropdown
  4. Select the Chart Note Type you want to appear as the heading for that appointment’s chart note

Now, when you write a new chart note for that appointment type, the heading will automatically reflect the new Chart Note Type.
