The Review of System (ROS) questions can be customized to fit the needs of your specific practice.
Reordering Questions
- Go to Practice > Charting > Review of Systems
- Click on a question and drag it up or down to the desired place in the list
- Changes are saved automatically
Removing an Existing Question
- Go to Practice > Charting > Review of Systems
- Click on the question to be removed
- Uncheck the Active checkbox
- Click

Adding a New Question
- Go to Practice > Charting > Review of Systems
- Click

- Fill out the fields:
- Patient Text - This is the text the patient will see
- 'Yes' Text - This is the affirmative text that is added to chart notes when importing ROS responses.
- 'No' Text - This is the negative text that is added to chart notes when importing ROS responses.
- Category - The system the question is related to. The category does not display to patients.
- Set any optional triggers for the question. These triggers will only display the question for the specified demographics.
- Check the boxes, if desired, to automatically create Patients when patients answer 'yes' to a question or if the response should be added to the PMHx.
- Click

Restoring Inactive Questions
If a question is saved with the Active checkbox checked it is hiden. Inactive questions can be viewed by clicking on the
button.
To reactive an inactive question, click on it then check the Active box.
Sub Questions

Subquestions are questions that will display based on the response to the master question. They can be used to ask for more detailed information when a patient answers affirmatively or negatively to a question.
Adding a new Sub Question
- Go to Practice > Charting > Review of Systems
- Click the
on the Maser Question - Check the Active box
- Specify if the question should display when the patient answers Yes or No to the master question
- Fill out the fields
- Patient Text - This is the text the patient will see
- 'Yes' Text - This is the affirmative text that is added to chart notes when importing ROS responses.
- 'No' Text - This is the negative text that is added to chart notes when importing ROS responses.
- Check the boxes, if desired, to automatically create Patients when patients answer 'yes' to a question or if the response should be added to the PMHx.
Editing an Existing Sub Question
- Click on the
icon that will display on the master question - Click on the subquestion to be edited
- Make the desired changes, then click
