Description

Account Integration Setup

1. If practice already has a Knowtex acccount, skip to step 2. below. 
   1. Setup an account with Knowtex https://www.app-knowtex.com/register.  This process will activate a 7-day trial of Knowtex (practices should contact Knowtex to setup an ongoing, paid account).
      1. Complete and submit setup form as indicated
      2. Enter "4D EMR" in the "Organization" field
      3. Login credentials will be sent to the email address listed on the form
2. In 4D EMR, navigate to Practice>>Settings>>Integrations and select the Knowtex tile as shown in item (1.) in below image.
   1. Click the Enable button as shown in item (2.) in below image.
   2. Click the Add icon as shown in item (3.) in below image.
3. A Cobalt secure connection dialog pop-up will appear as shown in item (4.) in below image.
   1. Click Continue, and enter the Knowtex login credentials and click Submit shown in item (5.) in below image.
4. A message will appear indicating the Knowtex account was successfully linked to 4D EMR and then redirect to the main 4D EMR application.
5.  Click the Add Resource button, select the appropriate 4D EMR Provider schedule from the dropdown, then click the blue check mark to save the configuration as shown in the below image.

4D EMR - Knowtex Integration

1. Once connected to Knowtex and configured for a Provider, 4D EMR uploads all appointments for configured Providers to Knowtex.
   1. New and updated appointments are automatically uploaded upon saving (there can sometimes be a lag before appearing on Knowtex schedule)
   2. Additional uploads will occur every 20 minutes to sync any not-synced appointments.
   3. Provider conducts and records consultations using the Knowtex application.
      1. Knowtex AI converts the consultation into detailed, structured medical notes.
      2. After conversion, the provider may review, edit, and approve notes from the Knowtex application prior to upload to the patient charts in 4D EMR.
2. Knowtex generated medical notes will be added to the appropriate patient charts.
   1. Upload to 4D EMR of medical notes from Knowtex created the same day as the appointment will occur immediately upon clicking the Upload to EMR button in the Knowtex application.
   2. Notes will appear as completed, signed notes in the 4D EMR Clinical>>Read Notes section of the patient record.
   3. Status of 4D EMR appointments for downloaded notes will be updated to indicate Chart Note Completed.

Knowtex AI Consultation Conversion Templates

1. Knowtex currently has two available standard AI conversion templates (Visit Notes and Op Notes).  
2. Upon request, Knowtex and create other visit-specific templates based on 4D EMR appointment types, including non-clinical conversations.  Please contact Knowtex directly for options and pricing.
3. Knowtex determines which AI conversion template to use based on the Appointment Type from 4D EMR

For example:  Knowtex will be configured to use the same AI conversion template for all appointments  uploaded from 4D EMR with 4D EMR Appointment Type name "New Patient Consult".

The below appointment examples are both appointment type "New Patient Consult".  Knowtex will use the same AI conversion template for both appointments.

One appointment has a sub-type for Facelift and the other for Abdominoplasty.  Neither of these values, nor information entered in "Concern" or "Comments" or other information associated with the appointment will be considered by Knowtex when determining the AI conversion template and final content of the note.

If the provider wishes to have different AI conversion templates for Facelift and Abdominoplasty consults, additional 4D EMR Appointment Type should be created and used for scheduling those consult types.  Knowtex can then use different templates for the different appointment types.

4. 4D EMR Note Types and note headers for downloaded Knowtex medical notes are determined by the Chart Note Type set for each 4D EMR Appointment Type.

5. 4D EMR Chart Note Templates are distinct and different from the Knowtex AI conversion templates.  4D EMR Chart Note Templates are not uploaded to Knowtex by 4D EMR or used by Knowtex when generating medical notes.

Other Knowtex usage information and options

• Knowtex has developed a plastic surgery-specific AI model trained on tens of thousands of specialty notes.
•  Knowtex AI can customize/train to the clinician as they make any edits in Knowtex generated notes and save changes.
• Accessible via any device - web app, mobile (apple & android), iPad, etc.
• After clicking upload, clinician can automatically see next patient even while previous note is processing. Can also add recordings to a note already generated to add a dictation or additional conversation, this will regenerate the note and combine the audio to create one comprehensive updated note.
• For same day upload: occurs automatically  after clicking "Upload" and confirming on the second pop-up.
• For older notes (not same day)  < 90 days from appointment: batch uploads twice daily at 7am and 7pm pacific time.
• For notes > 90 days : contact Knowtex team to help upload
• Uploaded notes to 4D EMR are still visible, but locked
• Providers still have 24 hours to override the signed note in 4D EMR and make any changes after uploading.
• New Macros feature: available for insertable phrases at organization and additional ones for individual users.

Addtional Knowtex coding capabilities options - CURRENTLY NOT ENABLED FOR 4D. Available upon request.

• Knowtex has the capability to automatically generate ICD-10 primary and secondary diagnosis codes from conversations and complete notes. Codes are editable and can be modified.
• Billing guidance features are also available but not currently enabled for 4D EMR users.
• 4D EMR integration currently only ingests the note content, and does not include coding upload.
