Training
4D EMR staff will provide practice staff with training on 4D EMR specific functions of the Forms Editor, including how to upload, create new, or copy digital content from ASPS or 4D EMR Community libraries described below.
- Practice staff must already have basic word processing skills.
- 4D EMR staff will create/add up to 5 pages of forms for the practice as part of the training process.
- Creation/addition of additional forms, templates, and consents by 4D EMR staff will be billed at the rate of $150/hr.
- 4D EMR subscription includes access to digital content in the 4D EMR Community library.
4D Community Documents
The 4D EMR Community includes consents, chart note templates, and forms contributed by other 4D EMR user practices.
- 4D EMR has not vetted these templates, forms, or consents for usability.
- Practices should fully vet and edit any Community content before using with the practice.
ASPS Consents
4D EMR can provide practices with access to digital-only versions of the below ASPS Consent sets
- 2019, 2021,2024 English Consents and 2021 Spanish Consents.
- Practices must already own or purchase these versions from 4D EMR or ASPS.
- Practices may upload and configure for use in 4D EMR any ASPS Consents or other consents they may already own or have in their possession if in a Microsoft Word (docx) or rtf format.
