Cost of Data Migration

  • One (1) source system is included at no charge.
  • If data is coming from multiple systems, additional fees will apply.
  • Any applicable fees will be reviewed and approved by you before migration begins.

What Data Is Migrated

4D EMR will migrate the following patient record and scheduling data from your existing system into 4D EMR as long as they are delivered in a machine readable format:

  • Appointments
  • Patient demographics
  • Primary patient chart notes
  • Patient photos
  • Scanned patient documents (PDFs)

Some  originating systems provide the below data in a format that can be migrated or converted to PDF and attached to the patient record in 4D EMR.  

  • Allergy lists
  • Medication lists
  • Communication logs
  • Patient alerts

Any additional data types not listed above will be reviewed separately and may require an additional migration fee.


Data Format Requirements

To ensure a successful migration:

  • Data must be provided in a machine‑readable* format (Excel or CSV), or
  • In a standard export format from a system that 4D EMR already supports
  • XML and other mark-up formats are not acceptable formats

All patient records and related data must include unique patient identifiers.

*If a computer can import the data without a human retyping, visually interpreting it or adding identifiers or mappings, the data is machine‑readable.


What Is Not Migrated

For clarity and system integrity:

  • Financial and billing data of any kind are not migrated, including charges, payments, insurance, or balances.
  • Any data type or category not listed as included above, 

Your Responsibilities

You are responsible for:

  • Requesting your data from your current system/vendor
  • Covering any fees charged by that vendor to release or export your data
  • Delivering the data securely to 4D EMR

4D EMR will guide you through the process but cannot request the data on your behalf.
