Description
Built directly into 4D EMR, AI Scribe allows you to bring your laptop, phone, or iPad into the room with the patient, and, by listening to your conversation, the 4D Scribe will create a properly formatted clinical note. AI Scribe gleans the data from the conversation, reorganizes it into the correct order, and eliminates conversation that does not pertain to the medical encounter.
For those not interested in AI-generated chart notes, AI Scribe also offers standard dictation.
Pricing
During the free trial period, May 1, 2026 through May 15, 2026, 4D AI Scribe will be automatically enabled for clients with charting included in their subscription at no additional charge. To continue using 4D AI Scribe after the trial period has ended, please contact 4D EMR Support.

Using 4D AI Scribe
It is recommended to use a smartphone or tablet to access AI Scribe, as they are less bulky. While using AI Scribe to record a note, in either AI Scribe or Dictation mode, the device should not be used for other tasks. Closing or browsing away from the record window will end the recording and cancel the chart note creation.
- On Computer: Right-click on the appointment, hover over Patient, and select "AI Scribe"
On Mobile: Tap on the appointment, then use the contextual radial button to access the appointment menu and select AI Scribe
- If it is your first time using it, 4D AI Scribe will require you to accept terms and conditions

- Select which Mode of Scribe you would like to use

Generate a Note using 4D AI Scribe
- Select Record Patient Encounter
- Select the Note Type in the top dropdown

- Tap or Click on the Blue Record button

- Complete the patient visit. AI Scribe will record the conversation, organize the visit details into a cohesive note, and automatically remove any non-relevant information. If needed, you can pause and restart recording.

- When finished, click or tap "Stop & Generate Note"
- Once the recording has ended, you can either save the note as a draft or wait for it to process

- After a brief processing period, your note will be displayed. You can make any needed changes
- 'Save and Finalize' to sign and finalize your note
Dictate a Note using straight transcription
- Select Dictate a Note
- Tap or Click on the blue Record button

- Dictate your note. If needed, you can pause and restart your recording.

- When finished, click "Stop & Generate Note.
- Once the recording has ended, you can either save the note as a draft or wait for it to process

- After a brief processing period, your note will be displayed as dictated. You can make any needed changes, then click 'Save and Finalize' to sign and finalize your note.

FAQ
Q: Are the recordings of the visits stored?
A: No. The recordings are deleted once the chart note has been generated.
Q: Are the recordings of the visits being used to further train the AI?
A: No. The recordings are only used to generate the chart note.
Q: Can the finalized note be edited?
A: Yes! Once the note has been saved and finalized, it can be accessed from Patients > Read Notes just like a manually created note. The note can be edited for 24 hours. After 24 hours, the note will become locked. However, addendums can be created with any further edits.
Q: Can a new chart note format be uploaded?
A: Yes. Please contact 4D EMR Support for assistance.
